Does the code tell you exactly which part has failed?

Not on its own. It identifies the system and fault type; investigation of the live data narrows down the actual cause.

I cleared the code and it came back — why?

Because the underlying condition is still present. Clearing a DTC does not fix the cause, which is what the investigation is for.
